Referral
Upon receipt of a referral, information will be reviewed to ensure that Phelps Case Management is a good fit for the situation. Upon acceptance of the referral and agreement to our terms and conditions an assessment date will be arranged as soon as possible.
Assessment
An Initial Needs Assessment will be carried in line with the Rehabilitation Code 2015 out to evaluate and make recommendations to improve recovery with all physical, psychological and or cognitive symptoms. A person’s full psychosocial dynamic is considered, as is their physical environment. Recommendations and estimated costs and predictions are put forward for clinical input and case management supports and report shared ten days from assessment.
Input
Following funding approval Case Management input will allow for the sourcing of suitable experts to provide goal driven clinical input to address needs. This may include clinicians, therapists, domestic supports, recreational support, vocational rehabilitation and or housing or equipment specialists. Goals will be developed with the person and their multidisciplinary team and input will be facilitated to maximise recovery.
Review
As progress is made, specific goals will be changed to allow for achievable steps to success. As communication is key, routine updates and full Progress Reports are shared with all relevant parties (full notes disclosures also available to representatives).
Whether additional input is needed or services changed and adapted to a persons’s needs, Phelps Case Management ensure that everything is in hand to maximise recovery and support provided until case closure.

Multi Trauma & Specialist Case Management
Proficiency in working under both single and joint instruction under the Rehab Code 2016; multi-trauma case management services include Initial Needs Assessments and ongoing case management input to achieve a swift recovery back to independence through private and statutory services.
Using sound clinical knowledge and with decades of collective experience working in the field of brain injury recovery and psychological injuries; services start with an Initial Needs Assessment and tailored to the needs of the client throughout the pathway to recovery. Skilled in the use of Interdisciplinary working the neurobehavioral approach to addressing all symptoms through a ‘whole person’ view to address all cognitive and functioning needs as well as physical and psychological symptoms.
Through input all symptoms and the full impact of injuries are assessed and addressed to formulate person centred goals to improve both a person’s independence and quality of life following their injuries. Utilising an employment first philosophy, where appropriate vocational supports are included throughout case management input.
Vocational Services
Vocational Rehabilitation services are offered as stand alone service with external Case Managers, OT, Psychologists etc. or through our clinical case management services. Open to referrals from those who have are engaged in, or who have already completed their clinical recovery, Vocational Rehabilitation services look to support a return to work or meaningful vocation.
With the objective to have individuals independently job seeking an appropriate job goal/vocation - input can be suited to a range of individuals at varying stages of their own recovery path.
Training & Support
Coaching and support packages can be formulated to address ongoing professional development and case management skills to other professionals in the field.
Examples of trainings that can be offered including: Understanding Psychological Injuries, Using Therapeutic Outcome Measures, Understanding Challenging Behaviours, Verbal Deescalation Support
