Why choose Phelps Case Management?
Phelps Case Management was founded in 2020. With a wealth of knowledge and experience guiding recovering persons back to optimal health - specialist case management input with a vocational rehabilitation focus is what we provide.
Rather than do things the typical way, we created input designed around our clients’ needs from a clinically driven with a holistic factors built in to planning from assessment on. We want to make sure all giving input are a ‘good match’ for the injured person, both with their clinical experience but also their professional interactions.
A 360 approach
Our focus is also on ensuring the recovery journey is as smooth as possible for the injured person; we do this by considering all psychosocial elements to a person’s needs.
Input leads to meaningful outcomes for the injured person that’s why with goals being developed with the person in mind and routine reviews, we keep these in mind every step of the way. Following funding approval, clinical teams are then tasked to create step by step goals to work towards the outcomes that are meaningful to the client and that these are communicated each stage of the way to all parties involved.
Quality of Life indicators can be used to track and document the impact clinical input is having and to illustrate areas where improvement could be worked towards.

If we feel that upon reviewing a referral that an alternative specialised case manager would better meet a persons needs we will always be open about this. We have connections to specialist case managers in a variety of fields around the UK and are happy to share contacts as appropriate.

It is vital that we are using up to date best practices both within case management but also by ensuring that all our clinical input is in line with current recommended guidelines. To ensure this we engage routinely in training and attendance in relevant professional development opportunities.

But we also, wherever possible, take the extra step in considering that those giving input are ‘the right fit’ for the client.
